Become a Medicaid Provider:

Once you become a THW Doula, you will need to apply to become an Oregon Medicaid Provider

  • Get you NPI here
  • Find the applications here to apply as a: 
    • Doula Organization
    • Non-payable Doula (working within a hub/agency)
    • Payable Doula

To Recertify: 

Recertification occurs every three years.  You will need to submit an application and present 20 hours of continuing education. 

If it has been more than six months since your recertification has lapsed, you will need to take a competency test. 


Doula Competency Test is administered by the Oregon Doula Association.
